The following are requirements for physical examinations, immunizations, dental examinations and vision examinations for the 2022-2023 school year. All forms are located in the School Health Office and at the bottom of this article. Please click here for more detailed information.
Physical Examinations/Immunizations are due October 17, 2022. All children are required by Illinois law to have a physical examination and required immunization by a licensed physician prior to entrance into pre-kindergarten, kindergarten, and 6th grade.
A Diabetes screening is mandatory as a part of the physical health examination.
In conjunction with the health physical, a mandatory Lead screening is required for children aged 1-7 years old.
Dental Examinations are due May 15, 2022 . A dental examination is mandatory for entry into kindergarten, 2nd, and 6th grades.

New Student Registration Requirements
New Student Registration for the 2022-2023 school year begins on August 11, 2022. Registration takes place in all schools on Tuesdays and Thursdays from 10 a.m. until 2 p.m. Before you attempt to register please make sure that you have all required documents listed below. There are no exceptions to the requirements. Please begin preparing now by downloading, printing down, and completing the forms on the following links. Once completed, please take your completed forms to your respective building principal(s). Forms should be completed for each child if you have more than one child attending school.
